Public Health England (PHE) has commissioned ICF Consulting to undertake an evaluation of the implementation of the framework. They are reviewing implementation of the framework nationally and locally, with a particular focus on addressing inequalities in physical activity, and on the two domains of 'active society' and 'active environments.'
Your survey responses are anonymous and your participation in the survey is voluntary.

On behalf of Public Health England, we are inviting all professionals with an interest in physical activity to complete this survey on the implementation of the Everybody Active, Every Day (EAED) framework.

Professionals include commissioners, providers, academics, policy makers, healthcare professionals, politicians etc., within the public, private and voluntary and community sectors. We would be very grateful if you could spend 10 to 15 minutes to complete this questionnaire. The survey will close on Friday 9th February.
